Forensic Assertive Community Treatment

Assertive Community Treatment (ACT) is a comprehensive person-centered, compressive community-based practice that provides integrated treatment, rehabilitation, and support to individuals diagnosed with thought disorders. Individuals served in this program are often receiving probation service and/or are being released from a correctional facility at the time of referral.

Forensic Assertive Community Treatment (FACT) provides integrated mental health services treatment for individuals who have been diagnosed with thought disorders who are also involved in the criminal justice system. Referred individuals are on probation and/or being released from a correctional facility at the time of enrollment. 

Services include psychiatry, therapy, nursing, substance use disorder treatment, vocational services, case management, and peer recovery services. The team also partners closely with the individual and their probation officer to work through requirements of the criminal justice system, with a focus on reducing additional legal charges and reducing time spent incarcerated.  

Like traditional ACT Teams, the FACT team also partners with the individuals served and their natural supports to build the skills needed to manage their illness while also reaching their life goals. Common things teams help with include finding and maintaining housing, obtaining competitive employment, building social networks, and re-establishing relationships. Teams meet with people in their homes or community location of their choosing and provide services seven days a week, with crisis response services available 24 hours per day.

At intake, we will work with you to identify and understand any costs associated with your care.
