Services Team
A RADIAS Health and Health Partners Collaboration.
The Community Care Services Team (CCST) is a collaboration with one of the managed care organizations (Health Partners) and RADIAS Health. This program specializes in helping people with mental health and substance use disorders returning to, and remaining in, community settings after extended or frequent stays in inpatient hospitals or other similar settings. This program coordinates with the treatment team to provide services for individuals, supporting the development, writing, implementation, evaluation, and revision of overall treatment goals and plans in collaboration with the individual served. CCST also coordinates services with other county or community agencies including but not limited to local community hospitals, mental health case management, treatment providers, and primary care. Referrals come directly from Health Partners.
RADIAS Health strives to make mental health services available to all individuals. In addition to billing Medicaid plans, we partner with local Counties, the State, and Health Plans to make services accessible. At intake, we will work with you to identify and understand any costs associated with your care.
