The goal of Waivered Services Case Management is to assist adults on Medical Assistance Waivers in accessing services necessary to live in the least restrictive setting possible.

 

Case Managers assist in identifying services, medical equipment and/or home modifications needed to maintain community-based living in the least restrictive setting possible. Goal development and service planning utilizes Person-Centered principles, and incorporates both what is important to and important for the person.

 

Coordinated Services and Supports Plans are developed in collaboration with the person, locating and setting up services with providers in the community, and monitoring the quality of services. Case Managers can also assist with referrals for needed medical, social, educational and other services, regardless of the funding source.

 

Waiver eligibility is determined through a MnChoices Assessment. To access an assessment in Hennepin County call – (612) 348-4111
