Targeted Case Management
Targeted Case Management provides person-centered care coordination to individuals diagnosed with Serious and Persistent Mental Illness who are residents of Ramsey County. Case Managers help link individuals to needed services, which include mental health, chemical health, medical, social, vocational, legal, educational, and recreational services.
