Person-Centered Care is integral to how ACT Teams approaches their work. This training provides an overview of the Person-Centered Approach in supporting persons served within ACT. Learn about best practices for engaging the individual serviced, developing the plan from an inter-disciplinary team perspective, and translating the plan into the teams scheduled appointments.
Objectives
- Participants will have an understanding of person-centered planning within the context of ACT
- Participants will be aware of examples of person-centered language (as well as language to avoid)
- Practice using several tools to engage people in the planning process, translate the person’s goals into treatment plan language and understand how to carry identify interventions into routine service delivery.
